Credit Establishment Policy Acknowledgement
Indemnity and Hold Harmless Agreement
Guarantee
Policy Guide Receipt Acknowledgement
Photocopies of the following:
Current Official Business Certificate
New York State Dealer Bond
Valid Driver's License for Owner
Voided business check
Sales Tax certificate
In order to add representatives to your application, the following is required:
Completion of Authorized Representatives Addendum
Photocopy of valid driver's license for new representative
