The following forms must be completed:

• Central Auto Auction Dealer Registration Form
• New York State Resale Certificate
• Credit Reference Authorization Form
• Credit Establishment Policy Acknowledgement
• Indemnity and Hold Harmless Agreement
• Guarantee
• Policy Guide Receipt Acknowledgement

Photocopies of the following:

• Current Official Business Certificate
• New York State Dealer Bond
• Valid Driver's License for Owner
• Voided business check
• Sales Tax certificate

In order to add representatives to your application, the following is required:

• Completion of Authorized Representatives Addendum
• Photocopy of valid driver's license for new representative
